Patient Resources

New Patient Registration

During your first visit, we will do a complete medical evaluation, including a thoughtful review of your medical history and a comprehensive physical examination.

To make your first visit as easy as possible, we request that you complete our new patient forms listed below prior to your scheduled appointment. We also ask that you arrive 30 minutes early so we can complete registration and handle paperwork before you see your provider.

At your appointment, please be sure to update any changes in name, address, telephone, or cell phone. Please bring the following to your appointment:

Your medical record is confidential; we provide information to others only for treatment, payment, or healthcare reasons.

Financial Counseling

We assign every patient of Woodlands Medical Specialists a personal insurance coordinator to process claims and remain familiar with each account. We understand the burden of long-term medical expenses and will work with you to determine a payment plan that is right for your family.

If you are having difficulty making monthly payments on your balance, please contact your insurance coordinator. You might be eligible for referral to a helping agency or to our patient assistance coordinator. You can call your insurance coordinator during office hours at

Prescription Refills

For most prescription refills, please call your pharmacy directly and ask them to contact us. For certain pain medication, however, you must have a written prescription from our office. It is the policy of Woodlands Medical Specialists to refill pain medication during office hours only. We strongly urge you to request prescription refills several days before running out of medications.

Insurance & Billing

We earnestly strive to keep down health care costs and willingly process claims with all insurance carriers. Your personal insurance coordinator will process your claims and help you navigate the health care system. You can call your insurance coordinator during office hours by calling

Your insurance coverage is a contract between you and your insurance company. Please make us aware of all medical insurance policies that impact your health benefits, and always provide a copy of your most recent cards as they are issued. We appreciate your active participation in keeping us informed about your coverage, and we will work diligently to file your claims. We will also work with you to manage your healthcare services based on the benefits provided by your carrier(s).

Your statement will reflect insurance claims filed on your behalf, payments made by your insurer, payments made by you, and any outstanding balance. You are responsible for paying deductible charges, co-payments, and any additional balance not paid by your insurance company. Copayments are required at the time of service. We are happy to explain charges or answer questions about your statement.

Please make monthly payments on your account balance. Your cancelled check is a valid receipt for income tax purposes. If needed, we will assist you with additional special statements.

Emergencies

If you have an emergency, call 911 or go to the nearest emergency room. For urgent problems after hours, call Woodlands Medical Specialists and leave an urgent message with our answering service. Calls are immediately relayed to a physician, who will contact you as soon as possible.

We address urgent problems during office hours. Please contact us early in the day with medical concerns to provide our team with maximum time to address your needs. Our providers return calls to patients throughout the day and evening. We do everything possible to help you avoid an after-hours trip to the emergency room.
